Care coordinator
Location: Virtual
A care coordinator facilitates communication among physicians, advanced practice nurses, nursing facilities, patients and patients’ families. Responsibilities also include collecting nursing assessment data and data from patients’ medical records to complete H&P, progress notes and discharge summaries. The care coordinator also transcribes verbal orders from the physician or advanced practice nurse and works with billing patient visits. It is not necessary for a care coordinator to have healthcare experience, but it is important to have a heart for patients.
