Care Coordinator Responsibilities
Our care coordinators facilitate communication among physicians, advanced practice nurses, nursing facilities, patients and patients’ families. Collecting nursing assessment data and data from patients’ medical records to complete H7P, progress notes and discharge summaries is also part of the responsibilities. The care coordinator also transcribes verbal orders from the physician or advanced practical nurse and works with billing patient visits.
Previous healthcare experience is preferred but not required. We are looking for individuals with a positive attitude, strong work ethic and a heart for people.
